If you cannot complete your registration and medical history online you may print and complete these forms before coming in for your appointment. If any patient information has changed since your last visit (i.e. insurance carrier, address, etc.) please update your information online, or print and complete a new Patient Registration Form.

Steps For New Patients

The following steps will help you see the doctors and other care professionals at St. Joseph’s ENT Clinic

  • Do You Need A Referral?

    Some insurance plans require that you get a referral to our clinic, while others will let you make an appointment without one.

    1. Call your insurance to see if a referral is required.
    2. See your general practitioner first if necessary, or if you’re allowed to make an appointment without a referral, call us to make your appointment.
  • Make An Appointment

    Call our office at (208) 777-1320 to schedule an appointment.

  • Complete Your Medical History

    Complete your medical history and new patient paperwork via the emails you were sent upon scheduling your appointment.
